An initial therapy appointment is a 75-minute comprehensive session focused on understanding your history, concerns, and goals to create a personalized care plan.
An ongoing couples therapy session is a 60-minute appointment designed to strengthen communication, address challenges, and build a healthier relationship together.
A telehealth visit for medication refills, acute illness care, lab orders and reviews, preventive screenings, sleep and nutrition guidance, and referrals as needed.

Additional fee for scheduling a same-day primary care session for non-members - if available. (Waived for members)
Brief visit to monitor recovery and address any changes.
Comprehensive lab evaluation for testosterone, estrogen, progesterone, thyroid, and adrenal function, with results used to create a personalized hormone balance and optimization plan. A follow-up is included, along with a complimentary referral to our integrated specialist if indicated.
Hurt during training, exercise, or every day activity? Receive a rapid sports medicine assessment and receive the appropriate guidance including rehabilitation recommendations, and any indicated imaging or prescriptions.
Need a clearance form for schools, athletics, or extracurricular activities? Book a quick virtual visit to review your medical history, discuss any health concerns, and complete the required documentation so that you're ready to participate safely.

No-shows or missed appointments without 24 hour notice: fee must be paid before you will be allowed to reschedule.
Exceptions for true emergencies (family and medical emergencies).
This is not meant to be punitive, with less than 24 hour notice, the appointment time is not able to be filled by another patient and 24 hours before your appointment, time is spent preparing for your appointment. By scheduling an appointment you are consenting to this policy.
Additional Forms requested outside of appointments: $25 per form.

Medication Requests
Please do not have your pharmacy send refill requests on your behalf, as we do not process pharmacy-initiated requests.
All medication refills and changes require an appointment with your provider.
Controlled Substances
Controlled substances are prescribed only when we are your sole prescribing provider.
Follow-up visits are required at least every 30 days for active adjustments
Once stable, follow-up is required at least every 90 days
We limit the use of controlled medications, particularly benzodiazepines.
Prescription history may be verified through recent medical and pharmacy records.
If records are not available, treatment decisions will be based on clinical assessment and current guidelines.

Court-ordered or involuntary treatment
